If you would like a Covid Vaccine and you are a current patient, you can contact our office to schedule an appointment for one of our vaccine clinics or make an appointment at one of the pharmacies or your local Health Department.  We currently have limited supply of the Covid vaccines.

If you would like a Covid test due to symptoms or close contact, you will need to schedule an appointment for a virtual visit (ex. phone,tablet or computer with a camera) with one of our providers.  They will order a test to be sent to the lab of your choice.
